Dear Former Client,
I am moving my individual clinical services to a group practice in which I am a partner in. As part of this transition, treatment records will be maintained securely by me at The Mental Health Co-Op. These records will remain confidential and protected according to state and federal laws. Your records will automatically transfer with me to my new practice on November 15, 2025. If you prefer your records be sent to your current provider, please submit a written request by November 14, 2025.
